Please be advised that any use of the orders for competitive purposes by any person, licensee, or company shall be deemed grounds for administrative action by the Department of Insurance including revocation of any applicable licenses or certificates of authority.
When performing a search enter the data in the following formats:
- NAME for individual, enter the last name, then a space, followed by the suffix name or first name
- DOI # is the number that is assigned by the Department of Insurance; it should be entered as a six-digit number
- NAIC #/NAIC NPN #is the number that is provided by the National Association of Insurance Commissioners/National Insurance Producer
- LEGAL ORDER # is the number that is assigned by the Department of Insurance – Legal Team; it should be entered as a digit year followed by a dash and five-digit number (e.g., 2009-00001, 2009-00099, etc.)
