File a Complaint
There are different types of complaints that are accepted by the Department of Insurance. The Division of Consumer Protection investigates these complaints:
PLEASE NOTE: DUE TO PRIVACY AND HIPAA CONCERNS, COMPLAINTS CONTAINING MORE THAN ONE INDIVIDUAL INSURED PERSON WILL BE SUBJECT TO IMMEDIATE CLOSURE.

Consumer Complaint: (for private health insurance, life, auto, home, general liability)

A consumer complaint is filed by a consumer, or their representative, who is having difficulty settling a situation with an insurance company, agent, or adjuster.

Pharmacy Complaint:

A Pharmacy Complaint is filed by a Pharmacy Representative (Pharmacist, Third Party) or an Insured who wish to file a complaint against a Health Insurance Company or a Pharmacy Benefit Manager.

Provider Complaint:

A provider Complaint is filed by a medical provider against a health insurance company.
